I can't remember the last time I even had a period. Usually I just keep putting in a new ring to avoid having a period. But this last time I think I might have left it in for five weeks instead of 4...does this make it completely ineffective?

No. five weeks doesn't make it ineffective...if you leave it in over five weeks thats when it lowers its chances its protection of pregnancy.
